As a member of our Medical Home, you are provided with an on-call provider who can assist you after hours and on weekends. We encourage you to utilize us as your first call for any questions or concerns regarding your care. Our answering service will promptly connect you.
For your convenience, we have contractual arrangements with the following health plans.
- Anthem Blue Cross / Blue Shield
- Aspen Ski Company
- Cofinity (formerly known as Sloans Lake)
- Great West
- United Healthcare
We will bill those plans with which we have an agreement, and will collect any required co-payment at the time of service. If your health plan determines a service to be “not covered” you will be responsible for the complete charge. If that happens, we will send you a bill. Payment is due upon receipt.
If you have insurance coverage with which we do not have prior agreement, we will provide a SuperBill or HCFA 1500 form on an unassigned basis. In this case your insurer will send the payment directly to you. Therefore your charges for your care and treatment are due at the time of service. For all services rendered to minor patients, the adult accompanying the patient is responsible for the payment.
