Patient and Family Advisory Council (PFAC)

Do you have thoughts on ways Valley View could improve your experience as a patient or a caregiver? Have you ever wondered why Valley View does something a particular way? Would you enjoy engaging with Valley View and other patients to make change to the health care system?

If you answered “Yes!” to those questions, you may be perfect for our Patient and Family Advisory Council (PFAC).

What is the Patient and Family Advisory Council (PFAC) at Valley View?

The PFAC is a volunteer committee made up of patients and caregivers who assist a health care system like Valley View in understanding the patient experience. The committee includes the patient perspective in the development of everything from the wording of a survey to the layout of a new clinical space. The PFAC is made up of 6 to 12 patients and caregivers who can commit to participating for at least one year.

What does the PFAC do?

The PFAC meets nine times a year, mostly on a monthly basis. Meetings include:

  • Sharing feedback about experience as a patient and/or caregiver (only to the level of your comfort)
  • Presentations and discussions with Valley View staff about projects to ensure the patient perspective is considered
  • Hosting listening sessions with community partners to gather feedback from additional Valley View patients

What do I do if I’m interested in joining the PFAC?

Both Marija and Valeria can answer any questions you may have and can provide the basic application. Once you fill out the basic application, you may return it to either Marija or Valeria.

Why an application?

The PFAC is intentionally diverse to represent the full spectrum of patients Valley View serves. We seek patients from diverse parts of our service area, patients with different cultural backgrounds and experience and different stages of life and age. The application process assists in achieving that goal.

All patients and caregivers are welcome to apply. Should we have a large number of applicants representing specific portions of the patient population, some may be encouraged to apply in future years.
