PREPARE FOR YOUR VISIT

Prior to your visit it is important to bring along the following information  (if applicable):

  • Insurance card(s)
  • Medication list
  • Clinical notes
  • Radiology reports
  • Surgical notes
  • Injections reports
  • Physical Therapy notes

If you are new to our clinic, we suggest that you arrive 20 minutes early for registration.

How to find us:

The Spine Center is located at Valley View Hospital. We encourage patients to enter the Cancer Center Entrance, which is located to the right of the ER entrance. There is valet parking available most days. Take the elevator up to the 4th floor, you will take a left and our office is at the end of the hallway.
