Vaccine Attestation

You must meet the following requirements to receive your vaccine at Valley View Hospital on Wednesday, October 27:

  • You are 18 years or older.
  • You live or work in a high-risk setting, per the Centers for Disease Control.
  • It has been more than six months since you received your first dose of a COVID-19 vaccine.
